This document is required by the Department of Health and describes how we intend to accommodate in-house visitation. We are also required to share this document with our families. The second document is the Visitor Acknowledgement and Consent Form. Please review this form and share this form with your family members. You will be asked to sign this acknowledgement when you enter the facility to visit. Pleas
